Breakaway Transition Worker Requisition # 2248841 Facility Winnipeg Regional Health Authority Job Location Canada-Manitoba-Winnipeg Additional Location Details Main Street Project Job Stream Administration/Core Business Operations Job Type Permanent Salary/Rate $20.30 - $22.85 / Hour Position Status Full Time Employee Group MGEU EFT 1.0 Anticipated Shift Mon-Fri Days Number of Positions Open 1 Start Date of Employment ASAP Posting Date 01-Sep-2016 Expiry Date 01-Oct-2016 Educational Requirements N/A Languages Required English Job Description POSITION SUMMARY: Under the supervision of the Program Manager, and in accordance with MSP’s mission, goals, policies and procedures, the Transition Services Worker will provide holistic and comprehensive case management services to all clients including but not limited to: intake assessment, benefit assessment, goal setting, case plan development, strength/weakness/opportunities/threats of achievement of independence of activities of daily living, weekly case plan development meetings, progress monitoring, individual money management, tenant education, advocacy and referrals and provide supports for change that promote a healthy and independent life style. Using a flexible approach to services, the Transition Services Worker will implement strategies for independence and accountability for achievement of their case plan. The primary emphasis will be to empower clients to gain independence by strengthening self-reliance and community support. The Transition Services Worker functions as a member of a team in providing services designed to assist marginalised individuals through episodes of crisis, support them in meeting their basic needs and ensure their safety and well-being while involved with any MSP program and assist with capacity building. ESSENTIAL FUNCTIONS: Assist with preliminary intake with clients requesting other MSP services. Complete service documentation in accordance with MSP policies. Provide client care coordination, assessment, planning, goal setting, referral, crisis counselling (makes referrals for therapeutic counselling), advocacy and ongoing support services. Provide Life Skills support to clients through direct service (such as; teaching hygiene, banking, cleaning their room, using a stove, using a bus) to assist clients to develop personalized skills they will require to live independently in the community. Case Management – development of ‘Individual Client-Centered Plans’ and continue to evaluate client changing needs and monitor progress with a focus on housing, independence, and community support. Work closely with all staff at MSP and support systems to develop and maintain an integrated approach to providing services and supports to clients. Actively participate in the development and evaluation of responsive programming options for clients and where appropriate, accessing alternative courses of action. Act as the liaison with all involved service providers, including staff at MSP and those of outside agencies (such as; EIA workers, etc.). Respect the right of confidentiality in all matters affecting or involving MSP clients in accordance with The Personal Health Information Act (PHIA). QUALIFICATIONS: Grade 12 education and experience. Experience with outreach and case management Knowledge in the systems, addictions and mental health is required Ability to work with a client-centered care philosophy Education/Training and experience in Motivational Interviewing Excellent interpersonal skills An understanding/experience in working with culturally diverse populations is essential Training and experience in the housing first model Complex trauma training an asset ADDITIONAL REQUIREMENTS: Valid driver’s licence.
